To make your first visit faster and more efficient, choose the correct form for you or your child below, print it, and fill it out to bring to your appointment.
If you don’t get a chance to fill out the form ahead of time, don’t worry! Just make sure to arrive 10-15 minutes before your appointment time to complete them in our office.
What To Expect at Your Free Consultation
Your first visit is a big step toward the smile you’ve always wanted and we’re here to make it a great experience! You’ll be greeted by our friendly team, and likely by Joy, our office pup and resident greeter. True to her name, she brings a little extra happiness to every visit.
Please have your insurance information available in case you decide to start treatment after your consultation. Here’s what you can expect at your first visit:
- A warm welcome and personal tour so you feel right at home in our space
- One-on-one time with your treatment coordinator to talk through your smile goals and any questions you have
- A quick digital scan of your smile – no goopy impressions, just high-tech comfort
- A personal introduction to Dr. Tennison, who’ll walk you through your custom treatment plan
- A clear, friendly conversation about finances so you know exactly what to expect—no surprises
If you decide that it’s not the right time for treatment for you or your child, we’ll make sure to keep in touch and recommend observation if necessary.
Scheduling Follow-up Appointments
Visiting the orthodontist regularly is an important part of your orthodontic treatment. If you’ve chosen LightForce custom braces, however, your visits will be even less frequent! No matter which treatment you choose, our practice will work with you to ensure your appointment times are convenient for your schedule.

What if I Have an Emergency?
True orthodontic emergencies are unusual, but some issues can create discomfort. If you’re experiencing a problem with your braces, clear aligners, or retainers, call or text us at (817) 556-2323. We’ll help you solve the problem at home or schedule the earliest available appointment for you if necessary.
Visiting the Dentist During Orthodontic Treatment
Visiting the dentist once every 6 months is even more important when you’re going through orthodontic treatment. These routine teeth cleanings and dental checkups will keep your mouth healthy while you get your new smile!
If any extra dental care is needed as part of your treatment, we will coordinate with your family dentist to ensure you receive the best, most personalized care possible.
